How universal is this "green = safe", assumption, I wonder?

I can think of one notable contemporary exception. Divers use green regulator second stages (mouthpieces) for cylinders of pure oxygen. It is only safe to breath from these when quite shallow. Of course diving with cylinders of pure oxygen requires specialized training (beyond what your average diver like me has).
